List of Immune
Illnesses

All students attending Ontario schooling, in Ottawa, must have had their shots and immune against the following illnesses:

  • Diphtheria
  • Tetanus
  • Poliomyelitis
  • Measles
  • Mumps
  • Rubella

Immunization
Records

All international students must provide in full all the information requested in the immunization record form. The diligently filled out form must be signed and accompany the online admission documents. A copy of the student’s immunization records will be sent out for evaluation to the Program of Vaccination of Ottawa Public Health Department.

NOTE: The student may require additional vaccines once arrived in Canada
